Which filler (Sculptra, Radiesse, Voluma, etc.) for jawline?

shutterstock_85596703Q. I decided to get Sculptra treatments to create a more angled, masculine jawline as sort of very, preliminary 'try out' before thinking of jaw augmentation down the line. While the volume and width are very good after seven months and three treatments, is there any other filler (short of augmentation) that may more of a sharp, angled contour to the haw as opposed to a slight of amount of a bloat from Scupltra? Would I have to wait until the current Sculptra effect dissipates?

A. ​The use of Sculptra as a trial is a good idea particularly if injected under the dermis and along the bone as it will stimulate firming of the skin and growth of the bone.​
​During the next few months as the effects of Sculptra continue to stimulate and improve, Radiesse may provide the sharper, more angled contour you are looking for.
​
​Any one of these fillers can be used BUT as notice, they will not provide the same effect.
​
​For this reason, it is best to be evaluated by a well experienced and Board Certified Plastic Surgeon or ENT facial surgeon to examine and review your concerns.
​
​Considerations along the jawline include:
  1. ​Skin,soft tissue and fatty laxity creating an uneven jawline with jowling and / or weak angle.
  2. ​Lack of teeth causing the bony jaw to resorb providing less projection
  3. ​Angle of the jaw and balance of the jaw, maxilla-zygoma and temple regions
  4. Level of placement, action and projection from each filler
  5. Duration of each filler before your investment dissolves
  6. And other considerations

Lower Eye Rejuvenation and Cheek Lift without Surgery: Plastic Surgeon, Dr. Dean Kane of Maryland uniquely uses Radiesse and Chemical Peel to give a non-surgical eyelift.

It is perfect to build up the cheek mound, fill in the deep wells of the lower lids and camouflage the bags. Fat: While we may hate it on our hips it works great as a natural filler for Faces, Booties and Imperfections in the Body! Ask Dr. Dean Kane, Plastic Surgeon in Maryland How Fat Works.

Dr. Dean Kane, Plastic Surgeon of Maryland has been using fat in the most unique ways. Instead of discarding the fat after liposuction, Dr. Kane takes the patient’s fat and injects it into other areas of the body and face. Many patients love how the fat fills in their faces to give a beautiful upper cheek lift, fill in the nasal folds and plumps the lips. The big question is will it last? That will vary from patient to patient and depending where the fat is placed. Dr. Kane will tell patients that there is no guarantee as to how much will stay that is transferred and how long it will stay. But many of our patients will say that whatever portion does live it seems to stay for years and years. It is important to transfer the fat immediately after the fat has been liposuctioned (within the same operation) as the cells are still intact and require immediate blood supply from another part of the body for the fat cells to live.  Fat that has been frozen will not  last the same length of time. Fat is a larger molecular structure than the fillers like Restylane or Juvederm so it gives a fuller fill with greater plumpness. Be prepared: Fat transferred in the face will possibly cause greater bruising and swelling and will take a longer time for recovery, especially in the lips.

And no you can’t use another person’s fat. The patients who are lucky to be thin and don’t have enough fat to transfer are still lucky to have the other fillers like Restylane, Perlane, Juvederm Ultra and Radiesse to use in their faces that work just as well as fat.

So many patients ask two questions consistently: One, what is the difference between all the fillers? And two what is the difference between Restylane vs Perlane and Ultra vs Ultra Plus? The first answer is the same as the concept between Coke and Pepsi. As we described in an earlier blog for the difference between Botox and Dysport, the same applies here. It is two companies producing very similar filler products made out of hylauronic acid. Hylauronic acid is a natural substance found in our skin that gives us the plumpness in our face. When we age we tend to lose this plumpness and Dr. Kane uses these filler products to refill those areas of the face that have lost its fullness, such as the nasal folds, hollows under the eyes, marionette lines and any facial wrinkle lines.

The second question: What is the difference between Restylane and Perlane or Juvederm Ultra and Ultra Plus? The differences are basically the size of the molecular structure of the hylauronic acid gels. Both Perlane and Juvederm Ultra Plus are larger molecules which give a bigger volume fill and have been found to last longer from 8 to 12 months. Most patients opt for the larger gel size to achieve greater fill and better plumping with longer lasting effects.
