Could something like CoolSculpting or cryosurgery treat Gynecomastia? Can the cold kill glandular tissue? I'm not worried about fat.

Q.  I know cold at a certain temperature for a certain time can cause cell death in fatty tissue. I imagine it could do the same with any living cell. At what temperature would cells in your mammary glands die?
A.  ​ ​​Great question! ​CoolSculpting's cooling endpoint is just above freezing where well perfused tissues retain the heat to remain alive and where superficial fat is stimulated to die via apoptosis.

​Male breast tissue includes non-active tubules (which transfer breast milk) and no lobules (which make breast milk). This tissue is not easily penetrated by the cooling and appears to retain enough heat not to undergo a programmed cell death.

​If the endpoint goes below freezing, frostbite occurs and all tissues die within this frozen zone. This concept is used in cryotherapy for debulking or killing tumors in many areas of the body.  It is for this reason that the feed-back relays on the CoolSculpting machine are so important.

​There are several ways to remove such tissue depending on the amount of residual breast mass and the skills of the surgeon. These include:
  1. ​Lipocontouring with a unique cannula used for breast tissue
  2. ​Areola incision with a "grasp and cut" or full excision
  3. ​Breast mound excision with breast reduction techniques

Q.  Would cool sculpting work for me with 2 treatments? Or is Liposuction necessary? I am 37 years old and have stubborn arm fat which hasn't been improved with any type of exercise or diet all my life. Otherwise I am happy with my body. I am also nervous about the after effects of lipo. I have read many people saying that years after lipo their metabolism seems to shift and they start gaining weight uncontrollably in weird places. I have maintained a weight with 7 lbs of fluctuation through the last 15 years.

A.  ​​​Thanks for sharing your photos. Although they are limited, you seem thin with mild lipodystrophy (fat) and skin laxity of the upper arms. I can appreciate your frustration!
​It is best to obtain consultation from a well experienced, Board Certified Plastic Surgeon with all skills for a upper arm contouring and lift so you are not biased by their lack of options.

​Please consider the following:
  1. ​CoolSculpting 1 or 2 sessions per arm to reduce the superficial fat and possibly (although no guarantees!) obtain some skin shrinkage around the smaller volume. This is very limited to the areas bulging only.
  2. ​LipoContouring which will provide near circumfirential fat reduction and etching of the upper arm muscles. You may obtain some skin shrinkage around the smaller volume but also skin redraping due to the wider contouring.
  3. ​A arm(pit) tuck may help in advancing the skin and reducing the skin excess. This option may be performed at the same time as lipocontouring but in your case due to age and exercise may be best 6-12 months later after the lipocontouring has healed and softened. It limits the scar to the armpit but also limits the amount of skin reduction.
  4. ​A brachioplasty (arm lift). This incision comes down the inner aspect or back or the arm to remove skin excess but leaves a more visible scar. I would also recommend to do this a year later after lipocontouring.

Does CoolSculpting treat cellulite?

shutterstock_71938243Q.  Does CoolSculpting treat cellulite? My legs from the top down to my knees have bad cellulite and as I'm loosing weight it is looking worse. What can I do?

A.  ​​​Cellulite is nothing more than a superficial layer of normal fat cells caught beneath the poorly expansive skin. The fat appears bumpy and irregular because it is trapped between a skin ceiling, a tough unyielding fibrous floor and thin fiber walls. Any swelling or reduction in the metabolic fat will inflate or deflate its appearance on the skin. Like a parachute, more or less air will create dimples and pulls on the silk chute. A thicker skin such as those of black, Mediterranean, Asian, Indian and other origins will resist the irregularities on the skin. A thinner northern European skin will more easily show the pucker, dimpling, hollows and bulges.

​Sometimes, what you consider to be Cellulite may be skin laxity or other dermatologic concerns which are treatable.

​CoolSculpting is tried and true but limited in fat reduction with some skin contractibility on some patients. It is not found to be consistent or reliable in reducing the superficial fat that creates Cellulite. In fact, there is no reproducible or reliable technology to improve Cellulite. There ARE other alternative medicine and dietary options that may help.

​Please consult with a well experienced, Board Certified Plastic Surgeon or Dermatologist to evaluate and compare appropriate options for you.

Q.  I'm considering CoolSculpting for my arms as they have become saggy. Could it make them worse?

A.  CoolSculpting is a "tried and true" method to reduce fat content in skin bulges!
​It has worked very well on properly selected patients for the arms. In fact, even heavy patients have been pleased because the small increments of arm fat reduction allows them to wear tops with sleeves that have previously been tight.

Thinner patients are pleased with CS's results on their arms as well. Caveat emptor: do not expect skin reduction or tightening with CS! On occasion, patients, particularly of color or reddish coloration have experienced some skin shrinking. This is due to the inflammatory response which is seen more in these patients, than those of white, north european descent.
You should discuss CS as well as other options such as lipocontouring and / or arm lift with a well experienced, board certified Plastic Surgeon who has properly examined you.​

CoolSculpting vs. Liposuction Removal of Fat for the Arms from your Maryland Cosmetic Surgeon

Mommy Makeovers in Baltimore, Maryland

Now that your child has returned to school, you probably have a lot more free time to take care of  yourself. This could mean more frequent shopping trips or gym visits.  However, after having a child, some areas of your body are showing the after effects of pregnancy and breastfeeding . Thankfully, at Dr. Dean Kane’s Center for Cosmetic Surgery & Medi-Spa there is a way to improve those aspects of your body that show loss of volume and laxity in the breasts and excess volume and laxity in the tummy due to aging and pregnancy.  Some of the common procedures that mothers do as part of their ‘mommy makeovers’ are breast augmentation, breast lift,  a tummy tuck and liposuction and Coolsculpting.

Breast Augmentation
A breast augmentation is done with safe, natural silicone gel implants or saline implants, which are both FDA-approved. While saline and silicone breast implants both have an outer silicone shell, they differ in material and consistency. During pregnancy, many women loose some of their breast tissue and a breast augmentation can make the breasts appear fuller returning them back to their original shape and size.

Saline Breast Implants
A saline breast implant is filled with a sterile salt water. The implants are inserted into your breast empty, and then filled once they are in place to allow your breasts to look plump.

Silicone Breast Implants
A silicone breast implant is already filled with a silicone gel at the time of the surgery. Silicone gel mimics the feel of a human breast and some women find that they look and feel more natural than a saline breast implant.
Many women chose to get their breasts enhanced for many reasons from just being very small breasted, loss of breast tissue after pregnancy, to restore their aging and sagging breasts as well as for those women in the entertainment business.  A breast augmentation can be combined with breast lifts for those women who are also experiencing skin laxity and sagging of the breasts, the breasts need both lifting and filling. Women also combine breast augmentations with other cosmetic surgeries such as liposuction or tummy tucks to make sure that you look and feel your best.

Tummy Tuck
Traditionally, the Tummy Tuck or Abdominoplasty is performed when there is excess tummy skin and the internal muscles of the abdomen are stretched and distended (this can happen as the result of multiple childbirths or large size births). The surgery will dramatically narrow your figure forming an “hourglass “waistline. The excess fat and skin is removed while the abdomen is made tight and flat. The recovery from the traditional Tummy Tuck is generally 4 to 6 weeks. Women still need to establish a diet and exercise routine that maintains your new flatter abdomen. Patients are elated with the result of their flat, tight tummy.

Liposuction
Liposuction is another procedure that is used for removing excess fat from the body. It is ideal for when a large amount of fat needs to be removed and it is essentially suctioned out of the body (such as baby weight). Depending on the area of your body where the fat was removed from and how much fat was removed would determine how long the recovery period is. The full results of Liposuction begin to show a visible difference a few weeks after the procedure and continues over time to provide a thinner, more sculpted and contoured appearance.

Coolsculpting
Coolsculpting is also an ideal non-surgical procedure for women who have excess thickness from pregnancy, but can’t take the time out to recovery from a surgical liposuction procedure. Women want to get back into pre-pregnancy figure, but are also trying to care for their newborn children. Coolsculpting freezes unwanted baby fat in targeted areas so that women can feel better about themselves while they combine diet and exercise to get their bodies back to their original form. Coolsculpting would not be available right away for women who have had a C-Section as Coolsculpting can not be performed along a newly formed scar area.
